This form is used to comply with two
different exemptions from the prohibitions of the Code of
Ethics. A member of an advisory board may have his or her
conflict of interest waived by the appointing authority after
disclosure of the transaction or relationship using this form, as
provided in Sec. 112.313(12), Fla. Stat. Also, certain conflicts
of interest arising from transactions involving a sole source of
supply may be waived provided that disclosure of the conflict is made,
using this form, as provided in Sec. 112.313(12)(e), Fla. Stat. |
For advisory board members, prior to the
waiver. For persons seeking a sole source waiver,
prior to the purchase, rental, sale, leasing, or other business being
transacted. |
